1. Your payment receipt from this site.
2. Your Current National FOP Card (backside that contains info) or if you are an Associate Member of FOP 89, a copy of your current Associate Membership Card.
3. Your Maryland driver’s license.
4. Your Current Maryland registration with your name on it as the owner or co-owner.
5. And a copy of your current insurance card for this vehicle, with the policy number visible, and your name as the insured

Please make sure all of your documents are current and have all of the information requested.

We cannot process your application without the receipt and above documentation.
